Dr. Aaron Hager is a board-certified comprehensive ophthalmologist and the founder and physician-owner of River City Eye Associates in San Antonio, Texas. He established the practice in 2019 with a clear and deliberate philosophy: that every patient — regardless of insurance status, background, or complexity of their condition — deserves attentive, unhurried, dignified care. In a medical landscape increasingly shaped by volume-driven models and abbreviated appointments, River City Eye Associates was built as a principled alternative.

Dr. Hager sees patients across a full range of medical and surgical ophthalmology, including cataract surgery, glaucoma management, diabetic eye disease, macular degeneration, and comprehensive eye health maintenance. He is one of a select number of ophthalmologists in the San Antonio region offering the Light-Adjustable Lens (LAL) — a premium cataract surgery technology that allows the lens prescription to be customized with UV light treatments after surgery, giving patients an unprecedented degree of precision and personalization in their visual outcomes.

Training and Military Service

Dr. Hager completed his ophthalmology residency through the United States Air Force Joint Residency Program in San Antonio, Texas — one of the nation’s highly regarded military medical training pathways, which provides comprehensive surgical and clinical ophthalmology training across a broad and complex patient population. His military service gave him not only exceptional technical training but also the discipline, composure, and patient-first orientation that continue to define his practice today.

Following the completion of his military service commitment, Dr. Hager returned to San Antonio — a city he had come to know and love during residency — to build a practice rooted in the community he serves.

A Practice Built on Values

River City Eye Associates is not structured around maximizing patient volume or procedure throughput. Dr. Hager’s approach prioritizes thorough evaluation, clear communication, and sufficient time to understand each patient’s concerns, goals, and circumstances. Appointments are designed to allow patients to ask questions and leave with genuine clarity about their condition and their options — not simply a prescription and a checkout receipt.

This philosophy extends to how the practice handles referrals and care coordination. Dr. Hager works closely with primary care physicians, emergency medicine providers, endocrinologists, and neurologists throughout San Antonio to ensure that patients receive integrated, well-communicated care across specialties. He does not require optometric co-management relationships for referrals — the practice is open to all patients regardless of who referred them or who manages their routine vision needs.

Community and Mission

Beyond his clinical practice, Dr. Hager serves as a volunteer physician with iCare San Antonio, a faith-based organization that provides free medical care to homeless individuals, refugees, and uninsured patients throughout Bexar County. This work reflects a conviction that access to quality medical care — including eye care — should not be determined by a patient’s financial means or social circumstances. It is medicine practiced as ministry, and it remains one of Dr. Hager’s most meaningful professional commitments.

Technology and Innovation

Dr. Hager maintains an active interest in medical technology development, with a particular focus on accessibility solutions for patients with visual impairment inspired by a close long-time friend. This work reflects both his deep familiarity with the functional consequences of visual impairment and his belief that physicians are uniquely positioned to identify and address unmet patient needs through technology.

Serving San Antonio and Karnes County

River City Eye Associates operates two clinic locations: the primary San Antonio clinic at Oakwell Farms (3338 Oakwell Court, Suite 190, San Antonio, TX 78218), conveniently located near the intersection of I-410 and I-35 and accessible from Terrell Hills, Alamo Heights, Northwood, Cibolo, and surrounding communities; and a satellite clinic at Otto Kaiser Hospital in Kenedy, Texas, serving patients in Karnes County and the surrounding rural region who would otherwise face significant travel for ophthalmology care.

Dr. Hager is a member of the American Academy of Ophthalmology and maintains board certification through ongoing continuing medical education and examination.

River City Eye Associates accepts Medicare and most major medical insurance plans. The practice does not accept vision insurance, as ophthalmology is a medical specialty. A full list of accepted plans is available on the New Patient FAQ page.
